Si les coquillages sont considérés depuis longtemps comme des aliments à risque parce que consommés le plus souvent crus, les études épidémiologiques précises et complètes font défaut pour évaluer la part réelle des coquillages comme facteur de risque de certaines pathologies. Ce document décrit et discute les différentes approches méthodologiques d'une enquête épidémiologique dont le but est d'évaluer le risque d'hépatite A lié à la consommation de coquillages. Investigation of a hepatitis A outbreak due to oyster consumption in France in 2007 ArchiMer

Following the notification of nine hepatitis A cases partly clustered near an oyster growing area in the Côtes d’Armor district (Brittany, France), epidemiological, environmental and microbiological investigations were performed in order to identify the source and vehicle of contamination and devise control measures. The investigations identified 111 cases, all of whom lived or had stayed in the district. Of these cases, 87% had eaten raw bivalve molluscs and 81% had specifically eaten oysters. The raw molluscs originated from a single shellfish farm located at the north of Paimpol bay. The shellfish were probably contaminated while being stored in submersible tanks or purified in a land-based tank. They were tainted either by sewage overflows or releases...

The high incidence of Hepatitis A and B in institutionalized patients with Down Syndrome (DS) is not fully understood. Under poor hygienic conditions, immunological alterations might predispose individuals to these infections. Sixty three DS children between 1 and 12 years old living at home with their families were examined for anti-HAV and compared to age-matched controls (64 healthy children). This cross-sectional study was carried out from May, 1999, to April, 2000, at the Hospital de Clínicas of Porto Alegre, southern Brazil. This study evaluated the epidemiological behavior of the hepatitis A in Paraná state and compared the costs of the disease and the vaccination. This is an epidemiological descriptive study including a pharmacoeconomy analysis. We collected information in the national database reported cases (SINAN), in the mortality information system (SIM) and in the hospital information system (AIH) among 2000/2003 (Paraná State Public Health Department). We estimated the probability of one cohort of children to acquire hepatitis A during their lifetime and the costs with their treatment. In the village of Cavunge, located in a dry tropical, semiarid rural region of the state of Bahia, Brazil, a sentinel study on viral hepatitis is underway. We report on the first part of the study. The objective of this study was to determine the prevalence of serological markers for hepatitis A, B and C in the village. Cross sectional study. Blood samples were tested for serological markers of hepatitis A (HAV), B (HBV) and C (HCV) through ELISA-III assay. In HBsAg and anti-HCV carriers, HCV-RNA and HBV-DNA were checked by PCR. Acute liver failure complicating viral hepatitis A BJID

Hepatitis A is one of the most frequent infectious liver diseases affecting children worldwide. The disease is usually mild and self-limited, and complications are very rare. Nevertheless, hepatitis A can sometimes cause acute liver failure (ALF), a severe, life-threatening condition. Herein is reported a case of a child who presented ALF during a course of hepatitis A. The need for early identification of possible ALF cases among hepatitis A patients, and for effective ways of evaluating such a possibility, are discussed. We also emphasize the importance of prevention measures, especially vaccination.
