Diagnostic investigation of ventilator-associated pneumonia using bronchoalveolar lavage: comparative study with a postmortem lung biopsy BJMBR
Balthazar,A.B.; Von Nowakonski,A.; De Capitani,E.M.; Bottini,P.V.; Terzi,R.G.G.; Araújo,S..
The purpose of the present study was to validate the quantitative culture and cellularity of bronchoalveolar lavage (BAL) for the diagnosis of ventilator-associated pneumonia (VAP). A prospective validation test trial was carried out between 1992 and 1997 in a general adult intensive care unit of a teaching hospital. Thirty-seven patients on mechanical ventilation with suspected VAP who died at most three days after a BAL diagnostic procedure were submitted to a postmortem lung biopsy. BAL effluent was submitted to Gram staining, quantitative culture and cellularity count. Postmortem lung tissue quantitative culture and histopathological findings were considered to be the gold standard exams for VAP diagnosis. According to these criteria, 20 patients (54%)...

Effect of heat and moisture exchangers on the prevention of ventilator-associated pneumonia in critically ill patients BJMBR
Auxiliadora-Martins,M.; Menegueti,M.G.; Nicolini,E.A.; Alkmim-Teixeira,G.C.; Bellissimo-Rodrigues,F.; Martins-Filho,O.A.; Basile-Filho,A..
Ventilator-associated pneumonia (VAP) remains one of the major causes of infection in the intensive care unit (ICU) and is associated with the length of hospital stay, duration of mechanical ventilation, and use of broad-spectrum antibiotics. We compared the frequency of VAP 10 months prior to (pre-intervention group) and 13 months after (post-intervention group) initiation of the use of a heat and moisture exchanger (HME) filter. This is a study with prospective before-and-after design performed in the ICU in a tertiary university hospital. Three hundred and fourteen patients were admitted to the ICU under mechanical ventilation, 168 of whom were included in group HH (heated humidifier) and 146 in group HME. The frequency of VAP per 1000 ventilator-days...

Silva Júnior,João Manoel da; Rezende,Ederlon; Guimarães,Thaís; Campos,Edvaldo V. dos; Magno,Luiz André; Consorti,Lívia; Pereira,Renata Andréa Pietro; Nascimento,Maria de Lourdes; Mendonça,João Silva de.
Ventilator-associated pneumonia (VAP) is the most commonly-acquired infection in patients in intensive care units. We analyzed epidemiological and microbiological characteristics and the outcome, in a cohort of critically-ill patients with confirmed diagnosis of VAP. All patients who had been on mechanical ventilation (MV) for more than 48 hours were included in our study; material collection for microbiological analysis was done within the first 24 hours after beginning treatment or after changing antibiotics. There were 55/265 (20.7%) VAP cases diagnosed, at a rate of 21.6 episodes per 1,000 days of mechanical ventilation. Mean age of the patients was 66 years, with a mean APACHE II score of 26.7 + 7.0; male patients were more prevalent. The mortality...

Indications of a new antibiotic in clinical practice: results of the tigecycline initial use registry BJID
Curcio,Daniel; Fernández,Francisco; Cané,Alejandro; Barcelona,Laura; Stamboulian,Daniel.
Tigecycline is the first of a new class of antibiotics named glycylcyclines and it was approved for the treatment of complicated intra-abdominal infections and complicated skin and skin structure infections. Notwithstanding this, tigecycline's pharmacological and microbiological profile which includes multidrug-resistant pathogens encourages physicians' use of the drug in other infections. We analyzed, during the first months after its launch, the tigecycline prescriptions for 113 patients in 12 institutions. Twenty-five patients (22%) received tigecycline for approved indications, and 88 (78%) for "off label" indications (56% with scientific support and 22% with limited or without any scientific support). The most frequent "off label" use was ventilator...

Risk factors and evolution of Ventilator-associated pneumonia by Staphylococcus aureus sensitive or resistant to oxacillin in patients at the intensive care unit of a Brazilian University Hospital BJID
Moreira,Michel R.; Cardoso,Renata L.; Almeida,Alair B.; Gontijo Filho,Paulo P..
This study investigated the participation and risk factors of VAP by resistant (ORSA) or sensitive (OSSA) S. aureus to oxacillin and evaluated the implications of adequate or inadequate empirical antimicrobial therapeutics in its evolution in patients interned in a mixing ICU of adults. A patient control-case study with PAVs by ORSA and OSSA was carried out from May 2005 to April 2007 involving 993 patients. VAP was defined based on clinical, radiological, and microbiological (> 106 CFU/mL count in the tracheal aspirate) criteria. Four hundred and seventy four (47.7%) patients were submitted to mechanical ventilation with 141 (29.7%) VAPs, with S. aureus as the most frequent agent (41.2%). The phenotype ORSA accounted for 47.5% and OSSA for 52.5%,...

Ventilator-associated pneumonia agents in Brazilian Neonatal Intensive Care Units - a systematic review BJID
Silva,André Ricardo Araujo da; Silva,Thais Carolina da; Bom,Gabriel José Teixeira; Vasconcelos,Raissa Maria Bastos; Simões Junior,Robinson.
ABSTRACT Introduction Ventilator-associated pneumonia (VAP) is one of the most common healthcare-associated infections (HAI) in neonates admitted in neonatal intensive care units (NICUs). Methods We did a systematic review using PRISMA methodology to identify the main etiological agents in Brazilian NICUs. Eligible studies published without period restriction were identified in PUBMED, SCIELO, LILACS and DOAJ. Studies were included if they were conducted in neonates hospitalized at NICU. Studies done in outpatient care, neonates outside NICU, emergency department, primary care, long-term care facilities or a combination of these were excluded. Results We identified 6384 articles in the initial search and four papers met the inclusion criteria. In all...

Ventilator-associated pneumonia with Col-S strains: a successful comeback of colistin! BJID
Mukhopadhyay,C.; Krishna,S.; Vandana,K.E.; Shenoy,A.; Bairy,I..
Emergence of multi and pan-drug resistant Gram-negative bacteria causing nosocomial infections in intensive care settings has become a challenge for clinicians. The mortality rate of ventilator-associated pneumonia (VAP) is known to increase when the initial microbiological diagnosis and antimicrobial therapy are inappropriate. We present a case of a 18-year-old man, who after being admitted following an accident, had developed VAP due to multi-drug resistant Pseudomonas aeruginosa and Acinetobacter spp. and had a downhill clinical course despite broad-spectrum antibiotic treatment. The strains were found to be Col-S, as the susceptibility was tested. Colistin was instituted, with remarkable recovery. It is imperative to diagnose VAP with multi-drug...
