Disseminated histoplasmosis in HIV positive patients in Espírito Santo state, Brazil: a clinical-laboratory study of 12 cases (1999-2001) BJID
Casotti,Janaina Aparecida Schineider; Motta,Tânia Queiroz Reuter; Ferreira Júnior,Carlos Urbano Gonçalves; Cerutti Junior,Crispim.
Twelve cases of histoplasmosis in HIV-infected patients were found in a retrospective analysis at the Hospital Universitário Cassiano Antônio de Moraes of the Universidade Federal do Espírito Santo (HUCAM), Vitória (ES), from June 1999 to May 2001. The frequency of histoplasmosis among HIV-positive patients was 2.1% in the infectious diseases division of the hospital during this period. Histoplasmosis compromised mainly males (11/12), 27 to 44 years old, and residents of the metropolitan urban area (10/12). Alcohol abuse and tobacco smoking were described in 10 of the 12 patients. For all patients, this was the first opportunistic infection. Two of the 12 patients died; 10 patients had disseminated disease, one patient had an intestinal presentation and...

Aging with HIV: a practical review BJID
Cardoso,Sandra Wagner; Torres,Thiago Silva; Santini-Oliveira,Marilia; Marins,Luana Monteiro Spindola; Veloso,Valdiléa Gonçalves; Grinsztejn,Beatriz.
The worldwide elderly population is expected to grow by an additional 694 million people by 2025. By that time, there will be approximately two billion elderly people in the world, most of whom (80%) will be living in developing countries. Based on recent estimates, this population will number over 40 million in 2030 in Brazil and a consequent increase in governmental spending for this population can be expected. Since highly active antiretroviral therapy became available in the mid-1990s, the life expectancy of people living with HIV has increased significantly. Approximately 12 million life years were added to the world between 1996 and 2008 as a consequence of wider access to highly active antiretroviral therapy. In Brazil, the incidence of AIDS among...

Antimicrobial resistance and plasmid replicons in Yersinia enterocolitica strains isolated in Brazil in 30 years BJID
Frazão,Miliane R.; Andrade,Leonardo N.; Darini,Ana L.C.; Falcão,Juliana P..
Abstract Some studies evaluated the resistance profile of the Y. enterocolitica strains isolated in diverse countries. However, in Brazil the isolation and the study of Y. enterocolitica are not common and therefore information about the antimicrobial resistance profile of this species in this country is scarce. Therefore, the aim of this study was to evaluate the antimicrobial resistance of Y. enterocolitica of biotypes 1A, 2 and 4 isolated from clinical and non-clinical sources between 1979 and 2012, in Brazil. This study showed that some Yersinia enterocolitica of different biotypes remain susceptible to antimicrobials used for gastroenteritis treatment. Moreover, neither acquired resistance genes nor diversity of plasmids replicons were found; however,...

Seroprevalence of HIV among blood donors, antenatal women and other patients in a tertiary hospital in Nigeria BJID
Olajubu,F. A.; Osinupebi,O. A.; Deji-Agboola,M.; Jagun,E.O..
HIV/AIDS continues to remain a nightmare in the developing nations of the world especially in Nigeria, where about 2.9 million people are living with this problem. This study aimed at determining the sero-prevalence of HIV among both patients and blood donors at the Olabisi Onabanjo University Teaching Hospital, Sagamu, Nigeria. Samples were collected between January 1st, 2005 and December, 31st 2006 and were screened using two rapid test kits, with two different principles (enzymatic and agglutination). Samples positive to the two methods were taken as truly sero-positive. The seropositive rate among blood donors, antenatal women, in- and out patients were, 3.2%, 6.9% and 17.5% respectively. There were five cases of positivity to both HIV I and II....

Methicillin/Oxacillin-resistant Staphylococcus aureus as a hospital and public health threat in Brazil BJID
Carvalho,Karinne Spirandelli; Mamizuka,Elsa Masae; Gontijo Filho,Paulo P.
Methicillin-resistant Staphylococcus aureus is an established nosocomial pathogen (HA-MRSA, hospital acquired MRSA), but has recently begun to appear in the community (CA-MRSA, community acquired MRSA). The cause of resistance to methicillin and all other β-lactam antibiotics is the mecA gene, which is situated on a mobile genetic element, the Staphylococcal Cassette Chromosome mec (SCCmec). Seven major variants of SCCmec, type I to VII are distinguished. HA-MRSA disseminated worldwide and causes the majority of S. aureus nosocomial infections with a limited number of clones disseminated including the Brazilian Epidemic Clone (BEC, ST239-MRSA-III). CA-MRSA isolates are susceptible to non-β-lactam antibiotics, usually isolated from healthy individuals which...

Fusarium solani keratitis: role of antifungal susceptibility testing and identification to the species level for proper management BJID
Rosa,Priscila Dallé; Sheid,Karla; Locatelli,Claudete; Marinho,Diane; Goldani,Luciano.
ABSTRACT We report a patient with fungal keratitis caused by a multiresistant Fusarium solani in a tertiary care hospital located in southern Brazil. A 55-year-old man with a history of ocular trauma presented with keratitis in left eye. The patient has a complicated clinical course and failed to respond to local and systemic antifungal treatment, and required eye enucleation. Despite multiple topical, intraocular and systemic antifungal treatments, hyphal infiltration persisted in the corneal transplant causing continuous recurrences. The cultures of corneal biopsy scrapings were positive for Fusarium spp. The organism was identified to species level by multi-locus sequencing for translation elongation factor 1 alpha...

Prevalence of Staphylococcus aureus introduced into intensive care units of a University Hospital BJID
Cavalcanti,Silvana M.M.; França,Emmanuel R. de; Cabral,Carlos; Vilela,Marinalda A.; Montenegro,Francisco; Menezes,Daniela; Medeiros,Ângela C.R..
Staphylococcus aureus is one of the principal human pathogens that colonize healthy individuals in the community in general, and it is responsible for severe infections in hospitalized patients. Due to an increase in the prevalence of strains of methicillin-resistant S. aureus (MRSA), combating these microorganisms has become increasingly difficult. A descriptive study was carried out on 231 patients in intensive care at the Oswaldo Cruz University Hospital (HUOC) in Recife, Brazil between January and April 2003 to determine the prevalence of S. aureus and MRSA and to evaluate risk factors for colonization by these bacteria when introduced into Intensive Care Units (ICUs). Body secretions were collected from the nostrils, axillary and perineal regions, and...

Risk of tuberculosis among household contacts in Salvador, Bahia BJID
Lemos,Antonio Carlos; Matos,Eliana D; Pedral-Sampaio,Diana B; Netto,Eduardo M..
Tuberculosis is one of the most important infectious diseases in the world. Only 68% of the estimated new tuberculosis (TB) cases in Brazil are diagnosed. Our aim was to determine the risk of infection among household contacts. Study design. Cohort of tuberculin-negative household contacts followed for 12 Months. Methods. Household contacts of randomly selected index acid-fast bacilli (AFB)-positive TB cases were evaluated through clinical examination, thorax X-ray, tuberculin, AFB smear and culture. Contacts with a negative response to the tuberculin test (less than 10 mm diameter) were retested after 90 days. Tuberculin reversal (used as a parameter of infection risk) was defined as an increase of at least 10 mm from the last measurement. Results. 269...

Dyslipidemia in HIV-infected individuals BJID
Sprinz,Eduardo; Lazzaretti,Rosmeri Kuhmmer; Kuhmmer,Regina; Ribeiro,Jorge Pinto.
Metabolic complications continue to play a major role in the management of HIV infection. Dyslipidemia associated with HIV infection and with the use of combined antiretroviral therapy includes elevations in triglycerides, reduced high-density cholesterol, and variable increases in low-density and total cholesterol. The association between dyslipidemia and specific antiretroviral agents has been underscored. Multiple pathogenic mechanisms by which HIV and antiretroviral agents lead to dyslipidemia have been hypothesized, but they are still controversial. The potential clinical and pathological consequences of HIV-associated hyperlipidemia are not completely known, but several studies reported an increased risk of coronary artery disease in HIV-positive...

Expression of class 5 antigens by meningococcal strains obtained from patients in Brazil and evaluation of two new monoclonal antibodies BJID
De Gaspari,Elizabeth N.; Zollinger,Wendell D..
Determining the profile of antigen expression among meningococci is important for epidemiologic surveillance and vaccine development. To this end, two new mouse monoclonal antibodies (MAbs) have been derived against Neisseria meningitidis proteins (class 5). The MAbs were reactive against outer membrane antigens and were bactericidal. Selected anti-class 5 MAbs [(5.1)-3E6-2; (5.3)-3BH4-C7; (5.4)-1BG11-C7; (5.5)-3DH-F5G9 also 5F1F4-T3(5.c)], and the two new monoclonal antibodies C14F10Br2 (5.8) and 7F11B5Br3 (5.9), were then tested against different meningococcal strains, (63 strains of serogroup A, 60 strains of serogroup C (from 1972 to 1974); and 136 strains of serogroup B (from 1992) meningococci). Our results demonstrated that the expression of class 5...
