Polymorphism of the rDNA and tDNA loci in clinical isolates of Pseudomonas aeruginosa: a perspective for molecular epidemiology surveillance Genet. Mol. Biol.
Spacov,Isabel Cristina Guerra; Silva,Suzileyde Alberto Marques da; Morais Júnior,Marcos Antônio de; Morais,Márcia Maria Camargo de.
The Gram-negative bacterium Pseudomonas aeruginosa has a wide environmental and ecological distribution. It is an opportunistic pathogen that acquires resistance to multiple antimicrobial agents and can infect plants, animals and humans. We used rDNA and tDNA PCR markers to characterize the bacterial diversity of P. aeruginosa strains isolated at a Brazilian teaching hospital (Oswaldo Cruz University Hospital, Recife, Brazil) between March 2003 and February 2004. Clonal groups of P. aeruginosa clinical isolates were identified from different patients in different hospital units using either rDNA or tDNA markers, or a combination of both in a duplex PCR. These PCR-typing methods together with drug-resistance profiles were used to trace the distribution of...

Ants as carriers of antibiotic-resistant bacteria in hospitals Neotropical Entomology
Moreira,Denise D.O.; Morais,Verônica de; Vieira-da-Motta,Olney; Campos-Farinha,Ana E. de C.; Tonhasca Jr.,Athayde.
The presence of antimicrobial-resistant bacteria and carried by ants isolated from hospitals was investigated in Campos dos Goytacazes, RJ, Brazil. Three health institutions were visited from 2001 to 2002 and samples were collected within critical areas of medical care based on criteria of greater risk of patient hospital acquired-infection. Four ant species were identified, Tapinoma melanocephalum (Fabricius)(63.1%), Paratrechina longicornis (Latreille)(21.1%), Monomorium pharaonis (L.)(10.5%), and Solenopsis saevissima (S. Smith)(5.3%) carrying 21 species of bacteria. The tests for bacteria species identification and antimicrobial-resistance profile were carried out by using standardized kits and evaluated by automated equipment. Among the bacteria...

Fontana,Renato; Wetler,Rita M da C; Aquino,Renata S S; Andrioli,João L; Queiroz,Guilherme R G; Ferreira,Sônia L; Nascimento,Ivan C do; Delabie,Jacques H C.
Nosocomial infections bring a high risk to the health of hospital patients and employees. Ants are common organisms in Brazilian hospitals, where they can act as dispersers of opportunistic microorganisms in places they forage. The occurrence of multi-resistant bacteria carried by ants was analyzed in two public hospitals (HA and HB) in southeastern Bahia, Brazil. In these two hospitals 132 workers belonging to three ant species were collected. The bacteria associated to these ants were identified and their susceptibility to antibiotics was evaluated. More than half (57.3%) of ants collected in HA were associated with some kind of bacteria, with 26.7% of them being opportunist bacteria, while 84,2% of the ants from HB presented associated bacteria growth,...

Klebsiella pneumoniae outbreak in a cancer unit of a general hospital: predisposing factors and evaluation of the impact of intervention measures BJID
Silva,Cristiane Pavanello Rodrigues; Amarante,Jorge Manoel Buchdid; Lacerda,Rúbia Aparecida; Biancalana,Maria Lúcia das Neves.
We made a retrospective cohort study of a primary bloodstream infection outbreak in patients of a cancer unit in a general hospital, evaluated the impact of intervention measures and investigated the predisposing factors. The targeted predisposing factors were selected based on the medical literature. The data were treated with univariate analysis to calculate the relative risk, and statistical significance was set at p<0.05. The presence of a long-term totally-implanted central venous catheter appears to be a predisposing factor, while a peripheral venous catheter appears to have offered protection from infection. This is reinforced by fact that intervention measures controlled the outbreak, suggesting cross-contamination from a common source. These...

In vitro antimicrobial activity of linezolid tested against vancomycin-resistant enterococci isolated in Brazilian hospitals BJID
Reis,Adriana O.; Cordeiro,Julio C. R.; Machado,Antonia M.O.; Sader,Helio S..
The emergence of vancomycin-resistant enterococci (VRE) has been described recently in Brazil. This is in contrast to the USA and Europe, where the VRE appeared in the late 1980s. The progressive increase in VRE isolation poses important problems in the antimicrobial therapy of nosocomial infections. Treatment options and effective antimicrobial agents for VRE are often limited and the possibility of transfer of vancomycin genes to other Gram-positive microorganisms continues. In the search for antimicrobial agents for multiresistant Gram-positive cocci, compounds such as linezolid and quinupristin/dalfopristin have been evaluated. The present study was conducted to evaluate the in vitro activity of the oxazolidinone linezolid and 10 other antimicrobial...

SENTRY antimicrobial surveillance program report: latin american and brazilian results for 1997 through 2001 BJID
Sader,Helio S.; Jones,Ronald N.; Gales,Ana C.; Silva,Juliana B.; Pignatari,Antonio C..
The alarming emergence and spread of antimicrobial resistance among common bacteria threatens the effectiveness of therapy for many infections. Surveillance of antimicrobial resistance is essential to identify the major problems and guide adequate control measures. Several resistance surveillance programs have been implemented in North America and Europe in the last decade; however, very few programs have assessed antimicrobial resistance in Latin American countries. The SENTRY Antimicrobial Surveillance Program was initiated in 1997 and represents the most comprehensive surveillance program in place at the present time worldwide. The SENTRY Program collects consecutive isolates from clinically documented infections in more than 80 medical centers...

Nosocomial infections in brazilian pediatric patients: using a decision tree to identify high mortality groups BJID
Lopes,Julia M.M.; Goulart,Eugenio M.A.; Siqueira,Arminda L.; Fonseca,Inara K.; Brito,Marcus V.S. de; Starling,Carlos E.F..
Nosocomial infections (NI) are frequent events with potentially lethal outcomes. We identified predictive factors for mortality related to NI and developed an algorithm for predicting that risk in order to improve hospital epidemiology and healthcare quality programs. We made a prospective cohort NI surveillance of all acute-care patients according to the National Nosocomial Infections Surveillance System guidelines since 1992, applying the Centers for Disease Control and Prevention 1988 definitions adapted to a Brazilian pediatric hospital. Thirty-eight deaths considered to be related to NI were analyzed as the outcome variable for 754 patients with NI, whose survival time was taken into consideration. The predictive factors for mortality related to NI (p...

Chang,M.R.; Carvalho,N.C.P.; Oliveira,A.L.L.; Moncada,P.M.F.; Moraes,B.A.; Asensi,M.D..
Nosocomial infections (NI) result in considerably high mortality and morbidity rates, especially among pediatric patients. Considering current worldwide changes, information about the occurrence of pathogens and susceptibility tests are now seen as decisive for optimizing treatment. The purpose of this research was to determine the frequency of microorganisms, antimicrobial and genetic profiles, and risk factors associated with nosocomial infections in a teaching hospital in Campo Grande, Mato Grosso do Sul. From January 1998 to December 1999, 108 patients were characterized as having nosocomial infection, from which 137 pathogens were isolated. Identification and antimicrobial susceptibility was determined by conventional and automated techniques....

Risk factors for nosocomial infection in trauma patients BJID
Giamberardino,Heloisa Ihle Garcia; Cesário,Eliane Pereira; Carmes,Eliane Ribeiro; Mulinari,Rogério Andrade.
Several factors are implicated in the increased vulnerability of multiple trauma victims to infection, especially in intensive care-units (ICU). This cohort study was designed to report the incidence, the topography, the etiology and to identify the risk factors for infection in trauma patients admitted in an ICU. From January 2000 to December 2001, 416 trauma patients were admitted to the ICU for more than 24 hours, the mean length of stay was 9.3 days (range 2-65) and 188 (45%) patients developed a total of 290 NI. The most prevailing infections were pneumonia (49%), bloodstream (19%) and urinary tract infections (12%). The variables studied were: the demographic data, diagnosis on admission, site and mechanism of injury, type and number of surgeries,...

Pathogen frequency and resistance patterns in Brazilian hospitals: summary of results from three years of the SENTRY antimicrobial surveillance program BJID
Sader,Helio S.; Gales,Ana C.; Pfaller,Michael A.; Mendes,Rodrigo E.; Zoccoli,Cássia; Barth,Afonso; Jones,Ronald N..
BACKGROUND: Pathogen frequency and resistance patterns may vary significantly from country to country and also in different hospitals within a country. Thus, regional surveillance programs are essential to guide empirical therapy and infection control measures. METHODS: Rank order of occurrence and antimicrobial susceptibility of pathogenic species causing bloodstream infections (BSI), lower respiratory tract infections (LRTI), wound or skin and soft tissue infections (WSSTI), and urinary tract infections (UTI) in hospitalized patients were determined by collecting consecutive isolates over a specified period of time, as part of the SENTRY Antimicrobial Resistance Surveillance Program (SENTRY). All isolates were tested by reference broth microdilution....

Profile of antimicrobial resistance of bacteria isolated from cockroaches (Periplaneta americana) in a Brazilian health care institution BJID
Prado,Marinésia Aparecida; Gir,Elucir; Pereira,Milca Severino; Reis,Cleomenes; Pimenta,Fabiana Cristina.
Cockroaches carry microorganisms to sterilized materials, equipment and non-contaminated food in hospitals and in homes. Hence, they are regarded as major microbial vectors. We isolated and identified microorganisms from living cockroaches captured from a Brazilian health care institution and we determined the antimicrobial susceptibility profile of the isolates. Following collection, cockroaches were immobilized at 4ºC for 10 to 20 minutes, immersed in 0.8% saline solution and then homogenized. Next, the suspension was seeded in culture media: brain and heart agar infusion, MacConkey agar, Naito agar, and they were incubated for up to 48 hours at 37ºC. Sabouraud agar cultures were incubated at ambient temperature. The microbial colonies were analyzed with...

Auxiliadora-Martins,M.; Menegueti,M.G.; Nicolini,E.A.; Alkmim-Teixeira,G.C.; Bellissimo-Rodrigues,F.; Martins-Filho,O.A.; Basile-Filho,A..
Ventilator-associated pneumonia (VAP) remains one of the major causes of infection in the intensive care unit (ICU) and is associated with the length of hospital stay, duration of mechanical ventilation, and use of broad-spectrum antibiotics. We compared the frequency of VAP 10 months prior to (pre-intervention group) and 13 months after (post-intervention group) initiation of the use of a heat and moisture exchanger (HME) filter. This is a study with prospective before-and-after design performed in the ICU in a tertiary university hospital. Three hundred and fourteen patients were admitted to the ICU under mechanical ventilation, 168 of whom were included in group HH (heated humidifier) and 146 in group HME. The frequency of VAP per 1000 ventilator-days...

Phenotyping and genotyping methods applied to investigate the relatedness of Brazilian isolates of Enterobacter cloacae BJMBR
Darini,A.L.C.; Magalhães,V.D.; Levy,C.L.; Barth,A.L.; Coscina,A.L..
In order to evaluate the resolving power of several typing methods to identify relatedness among Brazilian strains of Enterobacter cloacae, we selected twenty isolates from different patients on three wards of a University Hospital (Orthopedics, Nephrology, and Hematology). Traditional phenotyping methods applied to isolates included biotyping, antibiotic sensitivity, phage-typing, and O-serotyping. Plasmid profile analysis, ribotyping, and macrorestriction analysis by pulsed-field gel electrophoresis (PFGE) were used as genotyping methods. Sero- and phage-typing were not useful since the majority of isolates could not be subtyped by these methods. Biotyping, antibiogram and plasmid profile permitted us to classify the samples into different groups...

Carbapenem-resistant Acinetobacter baumannii outbreak at university hospital BJM
Takagi,E.H.; Lincopan,N.; Cassettari,V.C.; Passadore,L.F.; Mamizuka,E.M.; Martinez,M.B..
Nineteen clonally related imipenem-resistant Acinetobacter baumannii isolates were recovered from eight intensive care unit patients. All isolates harboured blaOXA-51-like β-lactamase genes and showed the absence of 22 kDa fraction in outer membrane porin profile analysis. It suggests a combination of two mechanisms as responsible for carbapenemresistant phenotypes.
