Cytomegalovirus and Toxoplasma gondii seroprevalence in a Brazilian liver transplant waiting J. Venom. Anim. Toxins incl. Trop. Dis.
Almeida,R. A. M. B.; Silva,G. F.; Llanos,J. C.; Winckler,C. C.; Gomes,M. R. B.; Biagioni,D. S.; Paula,R. C. C.; Meira,D. A.; Bakonyi,A..
Cytomegalovirus (CMV) disease is a major cause of morbidity and mortality in solid organ transplantation. Disseminated toxoplasmosis after liver transplantation is a rare but fatal event. Serologic screening of the donor and the recipient is essential to prophylactic management, early diagnosis and therapeutic strategies to minimize the consequences of these infections. The aim of the present study was to determine the seroprevalence of CMV and Toxoplasma gondii (TG) in a Brazilian liver transplant waiting list (LTWL). Serological data were collected from 44 candidates on the LTWL between May 2003 and November 2004. Serological investigation of antibodies IgM and IgG against CMV (anti-CMV) and TG (anti-T. gondii) was performed using fluorometry commercial...

Schistosomal liver fibrosis and hepatocellular carcinoma - case series of patients submitted to liver transplantation BJID
Filgueira,Norma Arteiro; Saraiva,Camilla Maria de Alencar; Jucá,Norma Thomé; Bezerra,Matheus Filgueira; Lacerda,Claudio Moura.
ABSTRACT Schistosomiasis affects approximately 207 million people in 76 countries. The association between hepatocellular carcinoma and Schistosoma mansoni infection has been investigated. Studies using animal models suggest that the parasite may accelerate the oncogenic process when combined with other factors, such as hepatitis C virus infection or exposure to a carcinogen. Herein, we report a case series of six hepatocellular carcinoma patients from Northeast Brazil, with negative serology for both hepatitis B and C virus, submitted to liver transplantation, whose explant showed evidence of schistosomal liver fibrosis. Since all patients enrolled in this study were submitted to liver transplantation, we were able to access the whole explanted liver and...

Li,Z.; Chen,J.; Li,L.; Ran,J.H.; Liu,J.; Gao,T.X.; Guo,B.Y.; Li,X.H.; Liu,Z.H.; Liu,G.J.; Gao,Y.C.; Zhang,X.L..
Hepatic oval cells (HOCs) are recognized as facultative liver progenitor cells that play a role in liver regeneration after acute liver injury. Here, we investigated the in vitro proliferation and differentiation characteristics of HOCs in order to explore their potential capacity for intrahepatic transplantation. Clusters or scattered HOCs were detected in the portal area and interlobular bile duct in the liver of rats subjected to the modified 2-acetylaminofluorene and partial hepatectomy method. Isolated HOCs were positive for c-kit and CD90 staining (99.8% and 88.8%, respectively), and negative for CD34 staining (3.6%) as shown by immunostaining and flow cytometric analysis. In addition, HOCs could be differentiated into hepatocytes and bile duct...

Survival benefits of interferon-based therapy in patients with recurrent hepatitis C after orthotopic liver transplantation BJMBR
Zanaga,L.P.; Vigani,A.G.; Angerami,R.N.; Giorgetti,A.; Escanhoela,C.A.F.; Ataíde,E.C.; Boin,I.F.S.F.; Stucchi,R.S.B..
Recurrent hepatitis C after orthotopic liver transplantation (OLT) is universal and can lead to graft failure and, consequently, reduced survival. Hepatitis C treatment can be used to prevent these detrimental outcomes. The aim of this study was to describe rates of hepatitis C recurrence and sustained virological response (SVR) to interferon-based treatment after OLT and its relationship to survival and progression of liver disease through retrospective analysis of medical records of 127 patients who underwent OLT due to cirrhosis or hepatocellular carcinoma secondary to chronic hepatitis C between January 2002 and December 2013. Fifty-six patients were diagnosed with recurrent disease, 42 started interferon-based therapy and 37 completed treatment....

Recurrent hepatitis C treatment with direct acting antivirals – a real life study at a Brazilian liver transplant center BJMBR
Zanaga,L.P.; Santos,A.G.; Ataíde,E.C.; Boin,I.F.S.F.; Stucchi,R.S.B..
Recurrent hepatitis C (HCV) after liver transplantation (LT) is an important cause of morbidity and mortality. Antiviral treatment is recommended to avoid unfavorable outcomes. Direct-acting antivirals (DAA) have transformed HCV treatment, with higher efficacy and fewer side-effects than interferon-based therapies traditionally used. To evaluate DAA treatment outcomes at a Brazilian transplant unit, data of patients who finished HCV treatment at the Liver Transplant Unit of the University of Campinas were analyzed. Treatment consisted of sofosbuvir, daclatasvir, and ribavirin, for 12 or 24 weeks, according to the national guidelines. Fifty-five patients completed antiviral treatment and 54 had HCV-viral load results available. The majority of patients were...

Occult hepatitis B virus infection in liver transplant patients in a Brazilian referral center BJMBR
Ferrari,T.C.A.; Xavier,M.A.P.; Vidigal,P.V.T.; Amaral,N.S.; Diniz,P.A.; Resende,A.P.; Miranda,D.M.; Faria,A.C.; Lima,A.S.; Faria,L.C..
Estimates of occult hepatitis B virus (HBV) infection prevalence varies among different studies depending on the prevalence of HBV infection in the study population and on the sensitivity of the assay used to detect HBV DNA. We investigated the prevalence of occult HBV infection in cirrhotic patients undergoing liver transplantation in a Brazilian referral center. Frozen liver samples from 68 adults were analyzed using a nested polymerase chain reaction assay for HBV DNA. The specificity of the amplified HBV sequences was confirmed by direct sequencing of the amplicons. The patient population comprised 49 (72.1%) males and 19 (27.9%) females with a median age of 53 years (range=18-67 years). Occult HBV infection was diagnosed in three (4.4%) patients. The...

Detection of human cytomegalovirus DNA in various blood components after liver transplantation BJMBR
Chen,X.Y.; Hou,P.F.; Bi,J.; Ying,C.M..
The quantification of human cytomegalovirus (HCMV DNA) by real-time PCR is currently a primary option for laboratory diagnosis of HCMV infection. However, the optimal sample material remains controversial due to the use of different PCR assays. To explore the best blood component for HCMV DNA surveillance after liver transplantation, whole blood (WB), serum (SE), and plasma (PL) specimens were collected simultaneously from targeted patients and examined for HCMV DNA using one commercially available assay. The HCMV DNA-positive rate with WB (16.67%) was higher than that with either SE or PL (8.33%, both P<0.01). Quantitative DNA levels in WB were of greater magnitude than those in SE (WB-SE mean log-transformed difference, 0.99; 95%CI=0.74-1.25;...

Fulminant hepatic failure caused by herbal supplies BJPS
Santos Júnior,Genário Oliveira; Ferreira Filho,Raymundo Paraná; Foulon,Liana Machado de Codes; Noblat,Lúcia de Araújo Costa Beisl.
The consumption of botanicals for therapeutic purposes has increased significantly in recent years. Drug-induced liver disease (DILI) is a frequent cause of acute liver injury, around 50% in the United States, and about 1% is secondary to the use of phytotherapeuticals and herbal supplies. Ruellia bahiensis, a plant species of the Acanthaceae family, is a tropical plant distributed in Northeastern Brazil. In folk medicine in the state of Bahia, the species is known as “mãe-boa” and is commonly used. L.S.S, a 23-year old, female, patient was admitted at University Hospital of Bahia-Brazil with signs and symptoms of acute hepatitis. She had made daily use of an herbal supply popularly known as “mãe-boa” for at least two years prescribed by a physician....
