Response to the complete hepatitis B vaccine regimen in infants under 12 months of age: a case series BJID
Miralha,Alexandre Lopes; Malheiro,Adriana; Miranda,Angélica Espinosa; Rutherford,George Williams; Alecrim,Maria das Graças Costa.
OBJECTIVES: Describing rates of seroconversion and its associated factors in a series of Brazilian infants following the final dose of the vaccine at 6 months of age. METHODS: Peripheral blood samples were collected after the third dose of the vaccine for the detection of anti-hepatitis B surface antibodies among infants of 7-12 months of age. We measured the association between seroconversion and birthweight, gestational age, time since administration of the vaccine in the maternity hospital and whether or not testing for hepatitis B surface antigen had been performed during pregnancy. RESULTS: We examined 40 infants. The mean birthweight was 2787 g (standard deviation = 853 g) and mean gestational age was 37.5 (standard deviation = 3.08) weeks. The...

Nasal CPAP in the delivery room for newborns with extremely low birth weight in a hospital in a developing country BJMBR
Gonçalves-Ferri,W.A.; Martinez,F.E..
The objective of this study was to determine the feasibility of the use of continuous positive airway pressure installed prophylactically in the delivery room (DR-CPAP), for infants with a birth weight between 500 and 1000 g in settings with limited resources. During 23 months, infants with a birth weight between 500 and 1000 g consecutively received DR-CPAP. A total of 33 infants with low birth weight were enrolled, 16 (48.5%) were females. Only 14 (42.4%) received antenatal corticosteroids and only 2 of those 14 (14.3%) infants weighing 500-750 g were not intubated in the delivery room, and apnea was given as the reason for intubation of these patients. Of the 19 infants in the 751-1000 g weight range, 9 (47.4%) were intubated in the delivery room, 6 due...

TNF-a and IL-10 levels in tracheobronchial lavage of ventilated preterm infants and subsequent lung function BJMBR
Corso,A.L.; Pitrez,P.M.C.; Machado,D.C.; Stein,R.T.; Jones,M.H..
The role of airway inflammation in ventilated preterm newborns and the risk factors associated with the development of chronic lung disease are not well understood. Our objective was to analyze the association of the airway inflammatory response in ventilated preterm infants by serial measurements of TNF-a and IL-10 in tracheobronchial lavage (TBL) with perinatal factors and lung function measured early in life. A series of TBL samples were collected from ventilated preterm infants (less than 32 weeks of gestational age) and concentrations of TNF-a and IL-10 were measured by ELISA. Pulmonary function tests were performed after discharge by the raised volume rapid compression technique. Twenty-five subjects were recruited and 70 TBL samples were obtained....

Application of continuous positive airway pressure in the delivery room: a multicenter randomized clinical trial BJMBR
Gonçalves-Ferri,W.A.; Martinez,F.E.; Caldas,J.P.S.; Marba,S.T.M.; Fekete,S.; Rugolo,L.; Tanuri,C.; Leone,C.; Sancho,G.A.; Almeida,M.F.B.; Guinsburg,R..
This study evaluated whether the use of continuous positive airway pressure (CPAP) in the delivery room alters the need for mechanical ventilation and surfactant during the first 5 days of life and modifies the incidence of respiratory morbidity and mortality during the hospital stay. The study was a multicenter randomized clinical trial conducted in five public university hospitals in Brazil, from June 2008 to December 2009. Participants were 197 infants with birth weight of 1000-1500 g and without major birth defects. They were treated according to the guidelines of the American Academy of Pediatrics (APP). Infants not intubated or extubated less than 15 min after birth were randomized for two treatments, routine or CPAP, and were followed until hospital...

Umbilical cord blood and neonatal endothelin-1 levels in preterm newborns with and without respiratory distress syndrome BJMBR
Benjamin,A.C.W.; Silveira,R.C.; Procianoy,R.S..
Increased pulmonary vascular resistance in preterm newborn infants with respiratory distress syndrome is suggested, and endothelin-1 plays an important role in pulmonary vascular reactivity in newborns. We determined umbilical cord blood and neonatal (second sample) levels of endothelin-1 in 18 preterm newborns with respiratory distress syndrome who had no clinical or echocardiographic diagnosis of pulmonary hypertension and 22 without respiratory distress syndrome (gestational ages: 31.4 ± 1.6 and 29.3 ± 2.3 weeks, respectively). Umbilical cord blood and a second blood sample taken 18 to 40 h after birth were used for endothelin-1 determination by enzyme immunoassay. Median umbilical cord blood endothelin-1 levels were similar in both groups (control:...
