Antibiotic prophylaxis in orthopedic surgeries: the results of an implemented protocol BJID
Queiroz,Raquel; Grinbaum,Renato S.; Galvão,Luciana L.; Tavares,Fernando G.; Bergsten-Mendes,Gun.
Though the basic principles of antibiotic prophylaxis have been well established, there is still considerable incorrect usage, including how much is prescribed and especially in the duration of treatment, which is generally superior to what is indicated. The adequate use of these drugs contributes towards decreasing the time of internment of the patient, prevents surgical site infection (SSI), decreasing the development of resistant microorganisms, and towards reduced costs for the hospital pharmacy. A protocol for the use of antibiotic prophylaxis in the Orthopedics and Traumatology Service of the Hospital do Servidor Público Estadual de São Paulo was developed. The objectives of the study were to promote rational antibiotic surgical prophylaxis, through...

Incidence of surgical site infection after craniotomy: comparison between three months and twelve months of epidemiological surveillance BJID
Torres,Silvana; Perdiz,Luciana Baria; Medeiros,Eduardo Alexandrino.
ABSTRACT Objective: To determine the incidence of surgical site infection in patients undergoing craniotomy and to compare 12-month and 3-month post-discharge surveillance periods in terms of their impact on the incidence of surgical site infection in those patients. Methods: This was a retrospective cohort study involving 173 adult patients submitted to “clean” craniotomy, with or without implants, during the six-month period, at a university hospital in the city of São Paulo, Brazil. All the patients were evaluated in the pre-, trans- and postoperative periods and were followed for 12 months to analyze the development of surgical site infections. Results: Of the 173 patients undergoing craniotomy during the study period, 20 developed an surgical site...

Surgical site infection in a university hospital in northeast Brazil BJID
Medeiros,Aldo Cunha; Aires-Neto,Tertuliano; Azevedo,George Dantas; Vilar,Maria José Pereira; Pinheiro,Laíza Araújo Mohana; Brandão-Neto,José.
We examined prevention of surgical site infection (SSI) in a tertiary teaching hospital in northeast Brazil, from January 1994 to December 2003. The survey included 5,742 patients subjected to thoracic, urologic, vascular and general surgery. The criteria for diagnosing SSI were those of the Centers for Disease Control, USA, and the variables of the National Nosocomial Infection Surveillance risk index were used. Data analysis revealed that anesthetic risk scores, wound class and duration of surgery were significantly associated with SSI. A total of 296 SSIs were detected among the 5,742 patients (5.1%). The overall incidence of SSI was 8.8% in 1994; it decreased to 3.3% in 2003. In conclusion, the use of educational strategies, based on guidelines for SSI...

Risk factors for surgical site infection following cesarean section in a Brazilian Women's Hospital: a case-control study BJID
Farret,Túlio Cícero Franco; Dallé,Jessica; Monteiro,Vinícius da Silva; Riche,Cezar Vinícius Würdig; Antonello,Vicente Sperb.
The present study evaluated patients with diagnosis of surgical site infection (SSI) following cesarean section and their controls to determinate risk factors and impact of antibiotic prophylaxis on this condition. Methods: All cesareans performed from January 2009 to December 2012 were evaluated for SSI, based on criteria established by CDC/NHSN. Control patients were determined after inclusion of case patients. Medical records of case and control patients were reviewed and compared regarding sociodemographic and clinical characteristics. Results: Our study demonstrated an association following univariate analysis between post-cesarean SSI and number of internal vaginal examinations, time of membrane rupture, emergency cesarean and improper use of...

Romanelli,RMC; Clemente,WT; Lima,SSS; Rezende,EM; Martinho,GH; Paiva,LFR; Neves,FAC; Madeira,JGC; Amâncio,GCS; Lima,AS; Faria,LC; Coutinho,RL.
Methicillin-resistant Staphylococcus aureus (MRSA) infections frequently complicate the post-operative course of transplant recipients, and despite nasal carriage and endemic colonization, MRSA outbreaks are not commonly described. This study reports a case of MRSA outbreak and discusses infection control measures and recommendations for this situation.
