Abstract Malaria elimination is now set to occur in Brazil until 2030. While this achievement is feasible, as it is for other endemic regions worldwide, it is important to recognize resistance of parasites and vectors against anti-malarial interventions. Resistance against drugs and insecticides can lead to discontinuities of malaria transmission, known as residual malaria transmission. Herein, we described a novel phenomenon that is occurring in a residual malaria transmission scenario in the southeastern Atlantic forest. An evaluation was made of the diagnostic efficacy and utility of the Parasight-F test in diagnosing Plasmodium falciparum malaria, compared with conventional microscopy, particularly in severe and complicated cases. This study was designed as a prospective, case control hospital-based study. Febrile patients suspected to be suffering from malaria were selected randomly and were subjected to peripheral smear examinations (thick and thin) and Parasight-F tests till the required number of at least 30 cases of P. falciparum infection were identified, including at least 15 complicated cases. Severe pulmonary involvement in malaria has been frequently reported in cases of Plasmodium falciparum infection, but rarely in vivax malaria. Among the 11 previous cases of vivax-related severe respiratory involvement described in the literature, all except one developed it after the beginning of anti-malarial treatment; these appear to correspond to an exacerbation of the inflammatory response. OBJECTIVE: To determine the sensitivity and specificity of the rapid diagnostic test OptiMAL® for diagnosis of Plasmodium vivax malaria. MATERIAL AND METHODS: We included all the patients who sought medical attention in the San Martin Pangoa Hospital, Junin, an area endemic for vivax malaria in Peru, between October and December 1998, who had fever during the previous 72 hours and who were older than 12 months. Falciparum malaria infection is associated with significant destruction of erythrocytes. This leads to the release of toxic metabolic products, including oxidant compounds.We measured the serum concentration of the antioxidant, ascorbic acid, in 129 patients presenting with acute falciparum malaria infection and in 65 healthy individuals. None of the study subjects administered any form of ascorbic acid supplementation within one week prior to participation in this study. The mean serum ascorbate concentration in infected adult males (n = 49, age range 18-50 years) was found to be 2.02 ± 0.20 mg/dL, and it was 2.03 ± 0.24 mg/dL in infected adult females (n = 56, age range 18-50 years).
