CREATING A CHILD FEEDING INDEX USING THE DEMOGRAPHIC AND HEALTH SURVEYS: AN EXAMPLE FROM LATIN AMERICA AgEcon
Ruel, Marie T.; Menon, Purnima.
Data from the Demographic and Health Surveys (DHS) for five Latin American countries (seven data sets) were used to explore the feasibility of creating a composite feeding index and to examine the association between feeding practices and child height-for- age (HAZ). Urban/rural differences were also examined. The data sets used were Bolivia, 1994 and 1998; Colombia, 1995; Guatemala, 1995 and 1999; Nicaragua, 1998; and Peru, 1996. The variables used to create the index were: breastfeeding (whether the mother is currently breastfeeding the child); use of baby bottles in the previous 24 hours; dietary diversity (whether the child received selected food groups in the previous 24 hours); food group frequency (how many days the child received selected food...

The use of operations research as a tool for monitoring and managing food-assisted maternal/child health nutrition (MCHN) programs: An example from Haiti AgEcon
Loechl, Cornelia U.; Ruel, Marie T.; Pelto, Gretel H.; Menon, Purnima.
This report describes the methods and results of an operations research undertaken to assess the effectiveness of World Vision (WV)’s food assisted maternal and child health and nutrition (MCHN) program in the Central Plateau region of Haiti. The research had three main objectives: (1) to assess the effectiveness of implementation and operations of the program relative to plans; (2) to assess the quality of delivery of the various services; and (3) to explore the perceptions of different stakeholders (i.e., beneficiaries and field implementers) regarding program operations and service delivery and the motivational factors that may affect staff’s performance and job satisfaction. The overall goal was to identify constraints to effective operations; it was...

WHY IS CHILD MALNUTRITION LOWER IN URBAN THAN RURAL AREAS? EVIDENCE FROM 36 DEVELOPING COUNTRIES AgEcon
Smith, Lisa C.; Ruel, Marie T.; Ndiaye, Aida.
While ample evidence documents that urban children generally have better nutritional status than their rural counterparts, recent research suggests that urban malnutrition is on the rise. The environment, choices, and opportunities of urbanites differ greatly from those of rural dwellers--from employment conditions to social and family networks to access to health care and other services. Given these differences, understanding the relative importance of the various determinants of child malnutrition in urban and rural areas--and especially whether they differ--is key to designing context-relevant, effective program and policy responses for stemming malnutrition. This study uses Demographic and Health Survey data from 36 developing countries to address the...

The GAPVU Cash Transfer Program in Mozambique: An Assessment AgEcon
Datt, Gaurav; Payongayong, Ellen M.; Garrett, James L.; Ruel, Marie T..
The GAPVU cash transfer program is an important safety net for urban Mozambique. The coverage of the program is impressive within the urban sector, reaching about 16 percent of all urban households. Although the mean transfer amount is just over a dollar per capita per month, it still represents about 13 percent of the beneficiaries' per capita consumption. Despite limited enforcement of means testing, nearly two-thirds of the beneficiary population are deemed to be absolutely poor by a modest poverty line. Net of GAPVU transfers, the proportion in poverty would have been above 70 percent. Limited evidence on nutritional and other nonconsumption indicators is suggestive of the GAPVU beneficiary households being more deprived than urban households in...

Plant Breeding: A Long-term Strategy for the Control of Zinc Deficiency in Vulnerable Populations AgEcon
Ruel, Marie T.; Bouis, Howarth E..
Because trace minerals are important not only for human nutrition, but for plant nutrition as well, plant breeding holds great promise for making a significant low-cost and sustainable contribution to reducing micronutrient deficiencies in humans, and may have important spinoff effects for increasing farm productivity in developing countries in an environmentally beneficial way. This paper describes ongoing plant breeding research that could increase the intake of bioavailable zinc from food staple crops among vulnerable populations in developing countries. The three most promising plant breeding strategies to achieve this goal are (1) increasing the concentration of zinc in the plant, (2) reducing the amount of phytic acid (a strong inhibitor of zinc...

ARE URBAN POVERTY AND UNDERNUTRITION GROWING? SOME NEWLY ASSEMBLED EVIDENCE AgEcon
Haddad, Lawrence James; Ruel, Marie T.; Garrett, James L..
The population of the developing world is becoming more urban. Are poverty and undernutrition beginning to relocate to urban areas as well? We use survey data on poverty (from 8 countries) and on child undernutrition (from 14 countries) to address this question. Using data from the past 15-20 years, we find that in a majority of countries the absolute number of poor and undernourished individuals living in urban areas has increased, as has the share of poverty and undernourishment coming from urban areas. Given these trends and the current stock of knowledge as to the levels, determinants, and solutions to urban poverty and undernutrition, we argue for more research on these issues.

GOOD CARE PRACTICES CAN MITIGATE THE NEGATIVE EFFECTS OF POVERTY AND LOW MATERNAL SCHOOLING ON CHILDREN'S NUTRITIONAL STATUS: EVIDENCE FROM ACCRA AgEcon
Ruel, Marie T.; Levin, Carol E.; Armar-Klemesu, Margaret; Maxwell, Daniel G.; Morris, Saul Sutkover.
This study uses data from a representative survey of households with preschoolers in Accra, Ghana to (1) examine the importance of care practices for children’s height-forage z-scores (HAZ); and (2) identify subgroups of children for whom good maternal care practices may be particularly important. Good caregiving practices related to child feeding and use of preventive health services were a strong determinant of children’s HAZ, specially among children from the two lower income terciles and children whose mothers had less than secondary schooling. In this population, good care practices could compensate for the negative effects of poverty and low maternal schooling on children’s HAZ. Thus, effective targeting of specific education messages to improve...

URBAN CHALLENGES TO FOOD AND NUTRITION SECURITY: A REVIEW OF FOOD SECURITY, HEALTH, AND CAREGIVING IN THE CITIES AgEcon
Ruel, Marie T.; Garrett, James L.; Morris, Saul Sutkover; Maxwell, Daniel G.; Oshaug, Arne; Engle, Patrice L.; Menon, Purnima; Slack, Alison; Haddad, Lawrence James.
No developing country can afford to ignore the shift in the locus of poverty, food insecurity, and malnutrition from rural to urban areas it is now experiencing. This review of recent literature explores the urban face of food and nutrition security in a more comprehensive, integrated way than most previous efforts. The review is organized around a conceptual framework that identifies food insecurity, inadequate caring behaviors, and poor health as the primary causes of malnutrition. It discusses current knowledge in eight areas that require the special attention of policymakers, development practitioners, and program administrators who wish to improve urban food and nutrition security: • the sources and cost of food; • incomes and employment; • urban...

DOES SUBSIDIZED CHILDCARE HELP POOR WORKING WOMEN IN URBAN AREAS? EVALUATION OF A GOVERNMENT SPONSORED PROGRAM IN GUATEMALA CITY AgEcon
Ruel, Marie T.; de la Briere, Benedicte; Hallman, Kelly K.; Quisumbing, Agnes R.; Coj de Salazar, Nora.
High urbanization rates in Latin America are accompanied by an increase in women’s participation in the labor force and the number of households headed by single mothers. Reliable and affordable childcare alternatives are thus becoming increasingly important in urban areas. The Hogares Comunitarios Program (HCP), established in Guatemala City in 1991, was a direct response to the increasing need of poor urban dwellers for substitute childcare. This government-sponsored pilot program was designed as a strategy to alleviate poverty by providing working parents with low-cost, quality childcare within their community. This paper presents preliminary findings from an evaluation of the HCP carried out in 1998 in urban slums of Guatemala City. The evaluation...
