Irregular root configurations which often challenge the requirements of successful root canal treatments are always misleading doctors to incorrect clinical judgments and treatment planning. In this article we describe a rare case of CBCT C-shaped mandibular premolar with a huge area of invagination measuring 6 mm ´ 3 mm, which we termed a "radicular rift valley" from a 76-year-old man. Considering the complex process of differential diagnosis, the overall status of disease condition and the will of the patient we proposed five treatment plans and finally conservatively chose plan B composed of both RCT and periapical scaling. Se han descrito forámenes ubicados en la cara interna del sector mandibular anterior, ya sea en hueso seco, radiografía convencional y en estudios mediante cone beam CT (CBCT). El objetivo de este estudio fue comparar la presencia, posición y las características anatómicas de los forámenes y canales linguales encontrados en CBCT de mandíbulas Chilenas secas, con los resultados de la misma muestra estudiada con visión directa en hueso seco. Se utilizaron 68 mandíbulas secas de la unidad de morfología de la Universidad de los Andes. The aim of this study was to assess the morphology of the mesial root canal system of maxillary molars and the frequency of MB2 canals using cone-beam computed tomography (CBCT). A total of 1374 teeth, first maxillary (1MS, n= 802) and second maxillary molars (2SM, n= 572) of 508 Chilean patients between 8 to 77 years were evaluated through CBCT. The mesiobuccal root was evaluated in all three thirds. Root canal morphology was classified according to Vertucci's method. Data were analyzed by Pearson's Chi-square and Cuzick trend tests. MB2 canal frequency in 1MS was 73.44% and in 2MS 42.48%. The most frequent morphology in 1MS and 2MS were Vertucci type II and I, respectively.
