Pneumonia severity index compared to CURB-65 in predicting the outcome of community acquired pneumonia among patients referred to an Iranian emergency BJID

OBJECTIVE: To compare the prognostic value of the pneumonia severity index and the severity score for community-acquired pneumonia (CURB-65) in predicting mortality and the need for ICU admission of patients with community-acquired pneumonia referred to our emergency department. MATERIALS AND METHODS: This prospective study was performed on patients with community-acquired pneumonia admitted to the emergency department of Imam Hossein Medical Center, Tehran, Iran. C-reactive protein in the diagnosis of community-acquired pneumonia BJID

Qualitative determination of C-reactive protein (CRP) was evaluated as a diagnostic method for community-acquired pneumonia. Paired serum and pleural fluid samples from child patients were examined with a CRP test, compared to bacterial cultures, counterimmunoelectrophoresis and immunoassay. The CRP test gave excellent parameters of sensitivity, specificity and predictive values for the diagnosis of bacterial pneumonia.

Community-acquired pneumonia in the childhood: analysis of the diagnostic methods BJID

Immunological assays such as CIE, LA, and Dot-ELISA were compared in order to diagnose community-acquired pneumonia. Serum, pleural fluid and urine samples were comparatively employed for bacterial antigen detection. Dot-ELISA proved to be an original and practical alternative procedure for detecting bacterial polysaccharide antigens from pleural fluid and/or concentrated urine samples, providing a rapid diagnosis for pediatric patients with community-acquired pneumonia.

Treatment of adults with community-acquired respiratory tract infections: results of a multicentric clinical trial with gatifloxacin BJID

Respiratory tract infections have an important clinical and economic impact and they are the most common indication for antibiotic use in outpatient practice. This prospective, multicenter non-controlled trial assessed the efficacy and safety of gatifloxacin in the treatment of community-acquired respiratory tract infections. Patients were treated with a daily oral dose of gatifloxacin 400 mg for 7-14 days. The diagnosis of respiratory infection was made based on the clinical condition and/or radiologic findings. Etiology of childhood community acquired pneumonia and its implications for vaccination BJID

Pneumonia is an important cause of morbidity and mortality among children throughout the world. Vaccines are available for some organisms, but they are underutilized and/or still in development. To evaluate the potential impact of vaccines, we review studies in which the etiology of childhood community-acquired pneumonia was recorded. In North America and Europe (9 studies), the etiology of pneumonia was established in 62% of studied children (range 43%-88%) by use of noninvasive specific methods for microbiologic diagnosis. The most often identified agents were S. pneumoniae (22%), respiratory syncytial virus (RSV) (20%), Haemophilus influenzae (7%), and Mycoplasma pneumoniae (15%). An open label, multicenter, non-comparative study of the efficacy and safety of oral gatifloxacin in the treatment of community-acquired pneumonia: a Brazilian study in five centers BJID

We made an open label, multicenter, non-comparative study to assess the efficacy and safety of oral gatifloxacin, 400mg PO given once-daily during 7 to 14 days for the treatment of adult outpatients with community-acquired pneumonia at five Brazilian medical facilities. Among the 86 subjects available for clinical evaluation, 84 (98%) were cured. The bacteriological eradication and presumed eradication rate was 98% (52/53) among the 44 (51%) patients who were bacteriologically evaluated. Drug-related adverse events were reported by 27% of the patients, diarrhea being the most frequent, occurring in 12% of patients. Adverse events were considered mild (89%) or moderate (11%). Rapid diagnosis of community-acquired pneumonia using the Bac T/ alert 3D system BJID

We compared BacT/Alert 3D with conventional culture for the diagnosis of community-acquired pneumonia (CAP). Antimicrobial susceptibility testing of the isolates was performed with the disk diffusion method, and the minimum inhibitory concentration (MIC) was calculated. Automation was superior in terms of recovery and time to detect pathogens. The bacterial spectrum in CAP was Streptococcus pneumoniae (35.3%) Staphylococcus aureus (23.5%), Klebsiella pneumoniae (20.5%) and Haemophilus influenzae (8.8%). Three of the 12 S. pneumoniae isolates showed penicillin resistance on MIC and two showed erythromycin resistance. Pneumonia is one of the leading causes of hospitalization and death among children in developing countries, and mortality due to pneumonia has been associated with S. pneumoniae infection. This investigation was designed to describe the antimicrobial susceptibility and serotype patterns of pneumococcal strains recovered from the blood of children with community-acquired pneumonia (CAP) and to assess the clinical findings of pneumococcal bacteremic patients with pneumonia. Success stories about severe pneumonia caused by Panton-Valentine leucocidin-producing Staphylococcus aureus BJID

We describe three cases of community-acquired necrotizing pneumonia which were caused by Panton-Valentine leucocidin-producing strains of Staphylococcus aureus (one of them methicillin sensitive). All cases were successfully treated without any sequelae for the patients due to the prompt initiation of adequate antimicrobial therapy. High suspicion toward this fatal pathogen was the key to the successful outcome of the patients.
