Diagnóstico de criptococosis a partir de lesiones cutáneas ABCL

Se realizó el diagnóstico de criptococosis en un paciente con lesiones cutáneas de aspecto moluscoide, presentes en gran número en el rostro, sospechadas clínicamente de ser provocadas por Histoplasma capsulatum. El paciente, posteriormente diagnosticado como VIH positivo, presentó además Cryptococcus neoformans en el LCR y fue tratado con anfotericina B endovenosa. Con material obtenido por escarificación de las lesiones se realizaron extendidos y practicaron cultivos. Aplicadas a los extendidos, las coloraciones de Giemsa, Grocott y mucicarmín de Mayer, permitieron la observación de levaduras redondas y capsuladas que en los cultivos en agar semillas de girasol desarrollaron colonias de color pardo que correspondieron a Cryptococcus neoformans.

Cryptococcus neoformans en heces de un paciente con SIDA ABCL

Se comunica el hallazgo de levaduras capsuladas en materias fecales recolectadas en forma seriada y remitidas al Laboratorio de Parasitología del Hospital Muñiz, para su examen específico. Paciente masculino de 49 años de edad, VIH positivo en el que se realizó el diagnóstico de meningoencefalitis por Cryptococcus neoformans mediante el examen del líquido cefalorraquídeo (LCR). El examen parasitológico de materia fecal realizado por haber presentado diarrea durante la internación, resultó negativo para enteroparásitos. Effect of pathogenic yeasts on human platelet aggregation BJID

We investigated the effects of Candida albicans, Cryptococcus neoformans and the respective culture supernatants on human platelet aggregation (PA). Both yeasts were unable to aggregate the platelets directly. On the other hand, cells of these yeasts significantly (P < 0.01) inhibited PA at concentrations equal to or higher than 1 x 10(6) cells/mL for C. albicans and equal to or higher than 1 x 10(5) cells/mL for C. neoformans. When the supernatants of one-week broth cultures were added to the activated platelets no inhibition in aggregation was observed. Antifungal susceptibilities of Cryptococcus species complex isolates from AIDS and non-AIDS patients in Southeast China BJID

Cryptococcus spp. are common causes of mycoses in immunocompromised patients. To determine the drug susceptibilities of clinical Cryptococcus spp. isolates, the characteristics of 61 clinical Cryptococcus spp. complex isolates and their antifungal susceptibilities were investigated, including 52 C. neoformans and 9 C. gattii isolates collected at Shanghai between 1993 and 2009. Central venous catheter-related bloodstream infection and Cryptococcus neoformans BJID

This is the first case reported of central venous catheter-related fungemia due to C. neoformans. A patient with chronic renal failure developed a fungemia during the treatment of a dialysis-associated bacteremia. Cryptococcus neoformans grew in the catheter tip and blood culture. We addressed questions about this catheter-related fungemia.

Multiple cutaneous swellings in an immunocompetent host: cryptococcosis overlooked BJID

Recently, there has been an increase in the incidence of cryptococcosis even among immunocompetent patients. We present a case of multiple subcutaneous swellings later developing into a non-healing ulcer caused by Cryptococcus in a 60 year-old immunocompetent male along with a brief review of clinical presentations of cutaneous cryptococcosis in immunocompetent and immunocompromised individuals. A high index of suspicion is desired in view of possible dissemination and variable non-specific clinical presentations.

Chronic skull osteomyelitis due to Cryptococcus neoformans: first case report in an HIV-infected patient BJID

ABSTRACT Osteomyelitis due to Cryptococcus neoformans are described in mostly 10% of patients with disseminated cryptococcosis, being direct inoculation even more uncommon. We report the case of an HIV-infected patient with history of recurring itching on his scalp and repetitive local trauma. For eighteen months, he noticed a painful and slow growing lump on his scalp. He was submitted to an excisional biopsy of the lesion but no etiological diagnosis was identified. After this procedure, the post-surgical wound never completely healed. Mortality and adverse neurologic sequelae from HIV-associated cryptococcal meningitis (HIV-CM) remains high due to raised intracranial pressure (ICP) complications. Cerebrospinal fluid (CSF) high opening pressure occurs in more than 50% of HIV-CM patients. Repeated lumbar puncture with CSF drainage and external lumbar drainage might be required in the management of these patients. The in vivo antifungal activity of the naphthoquinone beta-lapachone against disseminated infection by Cryptococcus neoformans was investigated. Swiss mice were immunosuppressed daily with dexamethasone (0.5 mg per mouse) intraperitoneally for 3 days, the procedure was repeated 4 days later, and the animals were then challenged intravenously with C. neoformans (10(6) CFU/mL) 1 week later. Seven days after infection, the mice were divided into groups and treated daily with beta-lapachone (10 mg/kg, iv) for 7 (N = 6) and 14 days (N = 10). Cryptococcus neoformans is an important opportunistic fungal pathogen that causes life-threatening infection of the central nervous system. A major virulence factor for C. neoformans is the production of melanin in the cell wall. Using transmission electron microscopy, we studied the cell walls of three pairs of isolates obtained from patients with dual cryptococcal infections, where a melanotic and an albino strain were isolated from the CSF of each patient. Susceptibility to heat and antifungal agents of Cryptococcus neoformans var. neoformans (serotype D) isolated from Eucalyptus spp in Rio Grande do Sul, Brazil BJM

In this work we studied the susceptibility to heat and antifungal agents of the first strains of environmental Cryptococcus neoformans var neoformans (serotype D) isolated in the state of Rio Grande do Sul, Brazil. In order to achieve a rigorous analysis, we employed the methodology recommended by NCCLS, Yeast Nitrogen Base (YNB) proposed by Ghannoum et al (YNB-1), Antibiotic medium 3 (AM3) indicated by others, YNB adjusted to the NCCLS methodology (YNB-2) and Etest. Our results indicate that all strains were susceptible to amphotericin B (0.0625-0.5 µg/mL), fluconazole (0.125-8.0 µg/mL), itraconazole (0.031-0.25 µg/mL) and flucytosine (0.125-4.0 µg/mL). In this study, we present antifungal susceptibility data of clinical and environmental isolates of Central Indian Cryptococcus neoformans (Serotype A, n = 8 and n = 50 respectively) and Cryptococcus gattii (Serotype B, n = 01 and n = 04 respectively). Susceptibilities to fluconazole, itraconazole and ketoconazole were determined by using NCCLS broth micro-dilution methodology. Different culture media applied to the study of Cryptococcus neoformans susceptibility to amphotericin B and fluconazole BJM

This study compared the performance of three culture media used for the determination of minimal inhibitory concentration (MIC) of amphotericin B and fluconazole against 48 Cryptococcus neoformans strains. For amphotericin B the media employed were RPMI 1640 medium, as indicated by NCCLS as reference, Yeast Nitrogen Base (YNB) as proposed by Ghannoum et al. (YNB-1), Antibiotic medium 3 (AM3) indicated by others, and YNB as proposed by us (YNB-2). Synergistic effect of ibuprofen with itraconazole and fluconazole against Cryptococcus neoformans BJPS

The present study investigated the association of the non-steroidal anti-inflammatory drug ibuprofen with itraconazole, fluconazole and amphotericin B against Cryptococcus neoformans isolates. The minimal inhibitory concentration (MIC) was found according to M27-A3 protocol and in vitro interactions were evaluated using checkerboard microdilution method. Synergism was demonstrated between azoles and ibuprofen for most isolates. However, no synergistic effects were seen when amphotericin B was combined with ibuprofen. Therefore, our results suggest that ibuprofen presents clinical potential when combined with azole drugs in the treatment of cryptococcosis.
