Patient and physician evaluation of the severity of acute asthma exacerbations BJMBR
Atta,J.A.; Nunes,M.P.T.; Fonseca-Guedes,C.H.F.; Avena,L.A.; Borgiani,M.T.; Fiorenza,R.F.; Martins,M.A..
We studied the ability of patients not experienced in the use of peak expiratory flow meters to assess the severity of their asthma exacerbations and compared it to the assessment of experienced clinicians. We also evaluated which data of physical examination and medical history are used by physicians to subjectively evaluate the severity of asthma attacks. Fifty-seven adult patients (15 men and 42 women, with a mean (± SD) age of 37.3 ± 14.5 years and 24.0 ± 17.9 years of asthma symptoms) with asthma exacerbations were evaluated in a University Hospital Emergency Department. Patients and physicians independently evaluated the severity of the asthma attack using a linear scale. Patient score, physician score and forced expiratory volume at the first second...

Dynamics of chest wall volume regulation during constant work rate exercise in patients with chronic obstructive pulmonary disease BJMBR
Takara,L.S.; Cunha,T.M.; Barbosa,P.; Rodrigues,M.K.; Oliveira,M.F.; Nery,L.E.; Neder,J.A..
This study evaluated the dynamic behavior of total and compartmental chest wall volumes [(V CW) = rib cage (V RC) + abdomen (V AB)] as measured breath-by-breath by optoelectronic plethysmography during constant-load exercise in patients with stable chronic obstructive pulmonary disease. Thirty males (GOLD stages II-III) underwent a cardiopulmonary exercise test to the limit of tolerance (Tlim) at 75% of peak work rate on an electronically braked cycle ergometer. Exercise-induced dynamic hyperinflation was considered to be present when end-expiratory (EE) V CW increased in relation to resting values. There was a noticeable heterogeneity in the patterns of V CW regulation as EEV CW increased non-linearly in 17/30 "hyperinflators" and decreased in 13/30...

Diastolic function is associated with quality of life and exercise capacity in stable heart failure patients with reduced ejection fraction BJMBR
Bussoni,M.F.; Guirado,G.N.; Roscani,M.G.; Polegato,B.F.; Matsubara,L.S.; Bazan,S.G.Z.; Matsubara,B.B..
Exercise capacity and quality of life (QOL) are important outcome predictors in patients with systolic heart failure (HF), independent of left ventricular (LV) ejection fraction (LVEF). LV diastolic function has been shown to be a better predictor of aerobic exercise capacity in patients with systolic dysfunction and a New York Heart Association (NYHA) classification ≥II. We hypothesized that the currently used index of diastolic function E/e' is associated with exercise capacity and QOL, even in optimally treated HF patients with reduced LVEF. This prospective study included 44 consecutive patients aged 55±11 years (27 men and 17 women), with LVEF<0.50 and NYHA functional class I-III, receiving optimal pharmacological treatment and in a stable clinical...

Dioctophymatosis as cause of dyspnea in a dog Ciência Rural
Mistieri,Maria Ligia de Arruda; Dill,Susane Werle; Rampelotto,Carine; Gomes,Etiele Maldonado; Pascon,João Paulo da Exaltação; Segatto,Thaline; Machado,Ingrid Rios Lima.
ABSTRACT: Dioctophyme renale is a parasite of the nematode class that can infect various species, including humans and dogs. Usually, the parasite migrates to the right kidney of the definitive host. Although, aberrant migrations have been previously reported, they mainly occur without clinical manifestations. No reports of dyspnea secondary to D. renale infestation has been found in the reported literature. The aim of this paper is to report intense respiratory distress caused by the presence of the parasite in the thoracic cavity of a dog. Radiographic images revealed multiple circular structures with a cavity with a radiopaque, thick contour in the thorax, which raised the suspicion of pulmonary bullae. Ultrasound examination revealed nematode...
