Patients with hepatitis C virus-related decompensated cirrhosis can benefit frominterferonbased antiviral therapy, but the common complication of cytopenia is a contraindication for this treatment. Splenectomy prior to interferon therapy may alleviate this problem. Severe fever with thrombocytopenia syndrome (SFTS) associated with severe fever with thrombocytopenia syndrome virus (SFTSV) is an emerging infectious disease. 12 patients with severe fever with thrombocytopenia syndrome in our study were presented mainly with fever and severe malaise. The clinical manifestations typically became worse on the 6th or 7th day. The average fever time is 9.11 ± 1.54 days. Most of them had multiorgan dysfunction, and part of them had hemophagocytic lymphohistiocytosis histiocytosis (HLH). Vancomycin has been used with increased frequency during the past 15 years and the most common toxicity with this drug is the "red man syndrome". Other adverse effects include neutropenia, fever, phlebitis, nephrotoxicity, ototoxicity, thrombocytopenia, interstitial nephritis, lacrimation, linear IgA bullous dermatosis, necrotizing cutaneous vasculitis and toxic epidermal necrolysis. Only two cases of vancomycin-induced Stevens-Johnson syndrome and one case of pancytopenia have been reported in the medical literature. Abstract Background Deinagkistrodon acutus envenomation is associated with severe hematological and wound complications but is rarely described. Case presentation Herein, we report three cases of victims bitten by D. acutus and indicate that rapid-onset severe coagulopathy and thrombocytopenia are distinct features of D. acutus snakebite, which are not observed in other crotaline snakebites (i.e., Trimeresurus stejnegeri and Protobothrops mucrosquamatus) in Taiwan. Immune-mediated thrombocytopenia, platelet destruction mediated by humoral and cellular responses, is frequently observed in dogs, especially in middle-age females of spaniels and Doberman breeds. The first line therapy involves the administration of corticosteroids in immunosuppressive doses and, in refractory cases, the pharmacological combination with other immunomodulatory agents seems often necessary. This case report describes the successful combination of vincristine sulfate to glucocorticoids (Dexamethasone) during the hospitalization of a dog with primary immune-mediated thrombocytopenia and related spontaneous bleeding, and discusses the therapeutic modalities in the literature. |
