PKO: alternative method for isolating mycobacteria from sputum Acta Amazonica

We elaborated an alternative culture method, which we denominated PKO (initials in tribute of respect to Petroff, Kudoh and Ogawa), for isolating Mycobacterium tuberculosis from sputum for diagnosis of pulmonary tuberculosis (TB), and to compare its performance with the Swab and Petroff methods. For the technique validation, sputum samples from patients suspected of pulmonary TB cases were examined by acid-fast microscopy (direct and concentrated smear), PKO, Swab and Petroff methods. We found that Petroff and PKO methods have parity in the effectiveness of M. tuberculosis isolation. The Brazilian State of Amazonas has a high incidence of Tuberculosis, 91.4 in 10,000 habitants (SESAU, 1994) and resistant strains of Mycobacterium tuberculosis are frequently being found in the region (SALEM et.al, 1990). These problems have been associated with side effects caused by the antibiotics used to treat Tuberculosis, which have in rum been associated with treatment non-compliance (PATTISAPU, 1984). The introduction of vaccination worldwide dramatically reduced the incidence of pathogenic bacterial and viral diseases. Despite the highly successful vaccination strategies, the number of cases among vaccine preventable diseases has increased in the last decade and several of those diseases are still endemic in different countries. Here we discuss some epidemiological aspects and possible arguments that may explain why ancient diseases such as, measles, polio, pertussis, diphtheria and tuberculosis are still with us.

ABSTRACT In Mexican Traditional Medicine 187 plant species are used in the treatment of respiratory conditions that may be associated with tuberculosis. In this contribution, we review the ethnobotany, chemistry and pharmacology of 63 species whose extracts have been assayed for antimycobacterial activity in vitro. Among these, the most potent is Aristolochia brevipes (MIC= 12.5 µg/mL), followed by Aristolochia taliscana, Citrus sinensis, Chrysactinia mexicana, Persea americana, and Olea europaea (MIC<64 µg/mL). This paper describes the in vivo Bronchoalveolar lavage (BAL) technique by endoscopy in tapirs (Tapirus terrestris) with clinical signs of tuberculosis. The technique was performed in two tapirs, male and female, from Curitiba Zoo, Paraná, Brazil. A flexible endoscope and a polyethylene catheter were used after the chemical restraint of the animals. The high pressure liquid chromatography (HPLC) was used for the identification of isoniazid (isonicotinic acid hydrazide) in the milk of cattle treated with a dose of 25 mg/kg/day in alternated days. The effect of milk pasteurization on the isoniazid residue concentration was also studied. The drug excretion presented a cyclic variation, with higher levels in the first day after administration (aa), a mean of 1104.48µg/l, and a decrease two days aa, with a mean of 104.12µg/l. Four days after the last administration of the drug it was not possible to identify residues of isoniazid in the milk of treated animals. The results obtained in evaluating the efficiency of a Neuro-Fuzzy System NEFCLASS (Neuro-Fuzzy Classification) in image classification of cattle tuberculosis, based on its texture features extracted using the wavelet transform are presented. For testing, images of animal tissues diagnosed with tuberculosis were used, as provided by the Tuberculosis Laboratory at the Instituto Biológico de São Paulo. The results of this study can serve as a basis for developing systems for diagnosis aimed at reducing human effort, by automating all or parts of the classification of images, helping lab technicians to sort amongst different pathologies.

Internal control in PCR for Mycobacterium tuberculosis: usefulness and improvement of the diagnosis BABT

The aim of this work was to construct and test a plasmidial Internal Control (IC) to detect the inhibition in the PCR test for M. tuberculosis and also its contribution for a Public Health Laboratory routine. The IC was a 600-bp of DNA linked to a plasmid with the same primer sites, allowing the amplification with the 245-bp diagnostic fragment. The amplification of the positive samples rendered the IC and the diagnostic fragment; instead negative samples only showed the IC. This study was performed considering the public health hazards related to the elimination of mycobacteria through milk of dairy cows suspected or positive for tuberculosis presenting no clinical alterations. A total of 780 milk samples from 52 animals, positive or suspected for tuberculosis, according to Stormont's test, were analysed to detect Mycobacterium spp. The samples consisted of 300 ml/cow, collected in the first milking of the day, during 15 days. The type of immune response induced by tuberculosis (Th1 or Th2) and its correlation with the clinical outcome is unclear. We studied 13 patients with active tuberculosis (TBC). The peripheral blood mononuclear cells producing IFN-gamma (PBMC-IG) were measured by enzyme-linked immunospot (ELISPOT) technique. We made a retrospective longitudinal study from January 2000 to January 2003 to examine cases of immune reconstitution syndrome (IRS) and its incidence rate in tuberculosis (TB)-human immunodeficiency virus (HIV) co-infected patients. The incidence rate (IR) was calculated using a Poisson regression. The confidence interval (CI) that was stipulated was 95%. IRS occurred in 10/84 HIV and TB-positive patients; nine of them were on highly active anti-retroviral therapy (HAART) during a mean of 61.7 (±59) days following the introduction of antiretrovirals. Study of the relationship between human MIF level, MIF-794CATT5-8 microsatellite polymorphism, and susceptibility of tuberculosis in southwest China BJID

OBJECTIVE: To study the human migration inhibitory factor (MIF) level in tuberculosis (TB) patients, and the relationship between MIF-794CATT microsatellite polymorphism and susceptibility of TB in Southwest China. METHODS: TB patients (n = 151) and healthy unrelated controls (n = 149) were recruited for this study. Genomic DNA was extracted, and then amplified by polymerase chain reaction (PCR). MIF-794CATT5-8 microsatellite polymorphism was genotyped by DNA sequencing. MIF level was detected by ELISA.
